To register for the Utah Suzuki Harp Institute, please download and print the appropriate registration forms (see links below). Complete all information required on the form(s) and mail completed form(s) with payment to: USHI, 356 Quail Nest Circle, Farmington, UT 84025. Alternatively, you may scan and email your completed form(s) to: registration@utahsuzukiharpinstitute.com

Registrants may pay by check or credit card. If you choose to payby check, please mail the check for the full amount due (payable to SAU) with your completed registration form(s). If you choose to pay by credit card, the Utah Suzuki Harp Institute, upon receipt of your registration form will then email an invoice which will enable you to pay via credit card through PayPal. Your spot atUSHI is not secure until payment is received. All credit card payments are subject to a 3% processing fee.
